Obviously way too soon for a definitive answer, however the Helo calling the traffic in sight may have been the first or second hole in the cheese.

257.6 is military UHF. VHF radios in airliners don't go that high.
Controllers broadcast on both so everyone an hear them. Civilians only hear half the conversation on VHF. Military transmit on UHF so we can't hear that side of the conversation. The ceiling is so the helicopters maintain clearance with aircraft, particularly in the landing phase. The Helos can easily operate below 200 feet in
VFR conditions. Their path also crosses the airplane runway traffic patterns. Subjects

See and avoid works well most of the time. It's the rest of the time that needs to be addressed. Normally one tower for fixed wing and another for helo traffic during busy hours at this field. One was apparently released early so the remaining on took on a high workload. That may have been the first hole in the cheese. Stepping back the whole scenario seems to be an accident waiting to happen. The Feds, local authorities or whomever should have dictated a minimum of two tower controllers during these high workload time frames.
