The recordings are simply of too low quality to draw any conclusions from.
If it was dual engine failure shortly after rotation, that is very bad luck and I doubt any crew could recover from something like that. I could speculate further but I think it\x92s just too hard to say without better quality footage or FDR data. I\x92m not a 787 driver so for fear of looking dumb in front of those that are this still confuses me. Even IF they\x92ve mis-selected the flap setting (I still don\x92t think it\x92s been cemented on here that there is in fact a FMS/flap setting disagreement warning but i believe there is), had the wrong de-rated take off settings, selected flaps instead of gear up the 787 with massive high bypass engines, FBW and full envelope protections surely cannot let itself be put in such a low energy/high alpha regime as we saw in the videos IF it has both fans functioning normally, surely? the pilots may have messed up royally and numerous times so those holes lined up but the plane is the final block in the chain and a 21st century all digital entirely clean sheet design was sold as being immune to such catastrophic outcomes from a few minor (consequential yes) and fairly common errors- aren\x92t all the protections and our procedures designed after decades of mistakes? im having a hard time squaring how a fully functioning modern bird like this could allow for this outcome and almost whatever the pilots did outside of unbelievable inputs and the pilots are are a bit of a red herring IMO of course like everyone else I have absolutely no idea what actually happened but I am willing to bet the explanation will be simpler than everyone is expecting. I am not going to speculate further as I believe it is a waste of time without further information. For the record I am not b787 rated but I do have Boeing and airbus narrow body and wide body time and i struggle to forsee a situation where the aircraft falls out of the sky at 200ft without a very serious error of some description. Imaginations are running wild for some people here. Major electrical fault that leads to double engine failure at v2? Really? If that were the case the 787 fleet would be grounded by now given the black box has almost certainly been examined.

I was thinking about this scenario when PF with the HUD down. The fuel switches are not in my field of view, so if you weren\x92t expecting it (and you wouldn\x92t be), a complete loss of thrust at v2 would come as a surprise to you as there are no engine indications on the HUD. I am not sure the PF would look down at the fuel switches in this situation, maybe at the EICAS but likely you will be busy flying the aircraft and leave the troubleshooting to the PM.
So if it were deliberate by the PM, the PF would be very slow to identify the problem. Unless you saw it happen (very unlikely), it would be virtually unrecoverable. what is the big deal about flight deck cameras? if it is treated the same way as the FDR then what is the problem?

Korea has made CCTV mandatory in operating theatres
.
I work in a facility where cameras outnumber staff on site roughly 3:1. Various forms of public transport (trains, buses, trams, I assume many boats) have ubiquitous external, internal, and in at least some cases driver facing CCTV. Plant control rooms (including power stations, dams, and chemical facilities) have ubiquitous CCTV. Driver-facing cameras with alerting and KPIs for events like speeding, hard braking, phone usage/distracted driving, and fatigue detection are becoming increasingly common for fleet vehicles, thanks largely to hefty insurer discounts. That's not just heavy trucks but things like HVAC installers, linemen, and ambulances. I suspect anyone who thinks this will never happen in aviation is being rather optimistic. CVR is likely what was meant. Subjects (links are to this post in the relevant subject page so that this post can be seen in context): CCTV CVR |
